The TSB has released its investigation report (M23C0305) into a cargo loss and fall overboard in Frobisher Bay, Nunavut, in 2023.
The investigation determined that the barge used by the cargo vessel Sivumut for transfer operations was operating beyond its stability limits, which compromised its ability to right itself.
The barge’s stability limits were unknown to the crew and were not readily available. Without this information, the crew relied exclusively on their past experience to determine the safe operating limits.
